为什么开展这项研究?

目前对非紧急眼科转进行分类的方法是主观的,并且取决于转介信中提供的患者资讯的品质。

白内障的诊断和分级需要裂隙灯生物显微镜和适当的训练。

深度学习模型可以作为临床助手,1)对非紧急专科眼科护理的新患者转介进行分类,2)仅通过视网膜照片的客观分析对白内障进行诊断和分级

 

这项研究是怎样进行的

第一阶段是使用现有资料库的43,274张视网膜照片对眼科转介和白内障严重程度分级进行自动深度学习系统的训练、调整和内部验证。

第二阶段为深度学习系统外部验证, 研究开始后收集数据,招募1290名中国研究对像, 2680只眼睛,年龄在50岁及以上,他们因视力模糊三个月或以上被转介至葛量洪医院眼科专科门诊。

 

这项 研究发现了什么?

我们开发的深度学习模型在外部验证测试中取得了很高的准确性,并且在新的非紧急转介和白内障程度进行分级, 对比眼科医生评估达到了中度至高度的一致性。

然而,深度学习模型也产生了比实际需求多 21.1% 的早期转介

Why did we carry out this work?

· Current methods to triage non-urgent ophthalmic referrals are subjective and dependent on the quality of patient information provided in referral letters.

· Cataract diagnosis and grading require a slit lamp biomicroscope and proper training.   

· A deep learning system can serve as a clinical aid to 1) triage new patient referrals for non-urgent specialist ophthalmic care and 2) diagnose and grade cataracts through the objective analysis of retina photographs alone.

 

How did we do this?

· The first phase was training, tuning and internal validation of an automated deep learning system to triage of ophthalmic referrals and grade cataract severity using an existing dataset of 43,274 retina photographs.

· The second phase was a prospective comparison study for external validation of the deep learning system, recruited 2680 eyes from 1290 Chinese subjects, aged 50 and above, who were referred to the Ophthalmology specialist outpatient clinic at Grantham Hospital for blurring of vision for three or more months.

 

What did we find?

· Our deep learning system achieved high accuracy in the external validation test and moderate-to-substantial agreement with ophthalmologist assessment for triaging new non-urgent referrals and grading of cataracts.

· However, the deep learning model also generated 21.1% more early referrals than necessary
